A care plan is one reading, ninety days of daily practice, and three honest re-measures against the day you started.
What the record keeps.
Your starting ratings, the chart evidence behind each selected priority, every scheduled practice, each kept or missed day, and every checkpoint answer stay together. The progress view compares the latest checkpoint with where you began without turning the comparison into a promise of an outcome.
- Day 0
The reading
Your answers and your chart produce a weighted profile, a resolution for each concern, and a remedy set filtered to the time you have.
- Days 1–30
Follow your scheduled practices
Each practice has its own record. Mark it when you do it; late completion keeps the original scheduled date.
- Day 30
First re-measure
The same questions again. The answers are compared with day 0, and the plan is adjusted to what actually held.
- Days 31–60
The second stretch
Your checkpoint reviews your answers and days kept. It may narrow the existing practice set so there is less to carry.
- Day 60
Second re-measure
Compare your current priorities with your earlier answers and review the days you kept.
- Days 61–90
The last stretch
Continue the practice set chosen at your latest checkpoint. Your earlier records remain available.
- Day 90
Close and compare
A final measure against day 0, kept in your account with everything that came before it.
Your record, not a proof.
The plan claims no outcome and gives you no score. It states what was set, what was done, what was missed and on which day, and it keeps all of it where you can read it back.
Asked most often
No. It ends on day 90 and nothing renews. What you did stays in your account to read back.
The misses stay on the record. At a checkpoint, your answers and days kept may lead to a smaller practice set. The plan is not extended.
At each checkpoint, your answers and days kept can narrow the existing practice set. This does not create a new chart reading.
No. It is part of the kept reading and is not billed again.
